Metabolic Health

Losing weight on GLP-1 without losing the muscle.

Studies show that 25–40% of weight lost on GLP-1 therapy can come from lean muscle mass. This is the most underaddressed risk in the GLP-1 era — and it is entirely preventable with the right protocol.

40%

of weight lost on GLP-1 therapy may be lean muscle mass without active preservation strategies. This worsens long-term metabolic health and increases frailty risk.

between body composition checkpoints. DEXA or BIA every 90 days ensures protocol adjustments before muscle loss accumulates.

PREVENTABLE WITH PROTOCOL

Muscle loss on GLP-1 is not inevitable. Protein targeting, resistance training, and select peptide stacking can preserve and rebuild lean mass during aggressive fat loss.

As Seen On

The Muscle Loss Problem

The weight loss is real. The muscle loss is the problem nobody is talking about.

GLP-1 agonists work by suppressing appetite — dramatically. The caloric deficit they create is significant. But when caloric intake drops without adequate protein and anabolic stimulus, the body catabolizes muscle tissue alongside fat. This is called sarcopenic obesity: you are lighter, but your ratio of fat to muscle has actually worsened.

Sarcopenic obesity is associated with worse metabolic outcomes, higher all-cause mortality, and dramatically increased frailty risk. It is the opposite of what GLP-1 therapy is supposed to achieve.

Protein requirements on GLP-1

Minimum 1.6g/kg body weight daily — and often more. Hitting this target is challenging when appetite is suppressed; a protein audit is essential.

Resistance training is non-negotiable

Without anabolic stimulus, muscle is catabolized for energy. Progressive resistance training 2–4 times per week is the floor, not the ceiling.

Creatine monohydrate

Evidence-supported supplement that reduces muscle protein breakdown during caloric restriction. Inexpensive, well-studied, and effective.

Peptide stacking options

BPC-157 supports tissue repair from training. Sermorelin or CJC-1295/ipamorelin can be combined where indicated to support lean mass during aggressive fat loss.

Benefits

What patients experience.

🏋️
Lean Mass Retention

Patients following our full muscle preservation protocol consistently retain lean mass during GLP-1 therapy, as confirmed by quarterly DEXA scanning.

⚡
Metabolic Rate Protection

Muscle drives resting metabolic rate. Preserving lean mass means your metabolism stays higher during and after weight loss, reducing rebound risk.

🔄
Long-Term Results

Patients who preserve muscle during GLP-1 therapy maintain weight loss more successfully after tapering or stopping the medication.

Our Protocol

Muscle preservation is not optional at Focus Lifestyle. It is built into every GLP-1 protocol.

We do not prescribe GLP-1 therapy without a muscle preservation plan. Before starting therapy, we establish baseline body composition with DEXA or BIA. Every 90 days, we recheck. If lean mass is declining, we intervene — adjusting protein targets, training guidance, or adding peptide support.

Baseline DEXA or BIA

Body composition is quantified before initiating therapy. Fat mass, lean mass, and visceral fat are tracked separately.

Protein audit

Daily protein intake is reviewed and structured to hit a minimum of 1.6g/kg body weight — even when appetite is suppressed.

Structured training plan

Progressive resistance training prescription matched to your starting fitness level and equipment access.

Quarterly reassessment

DEXA recheck every 90 days. If lean mass trend is unfavorable, the protocol is adjusted immediately.

How It Works

Your path to results.

01
Body Composition Baseline

DEXA or BIA before GLP-1 initiation. Fat mass, lean mass, and visceral fat quantified.

02
Muscle Preservation Plan

Protein targets, supplement protocol, and training guidance provided alongside GLP-1 prescription.

03
GLP-1 Initiation

Therapy begins with physician supervision, dose titration, and weekly check-ins during the first 4 weeks.

04
Quarterly Body Composition

DEXA recheck at 90 days. Results guide dose, protein, and training adjustments going forward.

GLP-1 therapy should leave you leaner and stronger. Not just lighter.
